Serious incidents: OSHA must be notified within 24 hours of an in-patient hospitalization, amputation or loss of an eye, and within 8 hours of a fatality. Those investigations are quoted separately. SafetyGrid does not take fatality investigations; please contact your legal counsel.
Example: Around 9:30 AM, a carpenter was installing ceiling grid from a 6 ft stepladder. He stepped onto the top step to reach. The ladder shifted and he fell to the floor, landing on his left wrist. The supervisor stopped work, gave first aid and took him to urgent care. The ladder was tagged out of service.
