Title IX Reporting Form

Anyone who seeks to report sexual harassment or other sexual misconduct that violates Meridian University’s Title IX Sexual Harassment and Sexual Misconduct Policy may file a report using this form. 

This report will be reviewed by Meridian University’s Legal Affairs Team, in coordination with the University’s Title IX Coordinator. Following review of your submission, a staff member from Legal Affairs will reach out to you to discuss the next steps.  

Following submission, the University will make reasonable efforts to respond promptly, consistent with applicable law and University policy. If you require assistance completing it, please email the Title IX Coordinator or Legal Affairs Team at: legalaffairs@meridianuniversity.edu.

Important Information About the Review Process

Consistent with applicable law, Meridian University seeks to resolve concerns at the earliest appropriate stage and encourages informal resolution when it is lawful, voluntary, and appropriate under the circumstances. Informal resolution will not be used when prohibited by law and will only be offered following an initial assessment by the Title IX Coordinator. Participation is voluntary, and either party may withdraw at any time and resume the formal grievance process.
