Participant Registration & Safety Agreement

Please read this document carefully before signing. This waiver applies to all Queer Combat Collective (QCC) training sessions, classes, and community activities.

By submitting this form, you confirm that you have read, understood, and agree to this waiver, and that you are 18 years of age or older. This waiver is governed by the laws of Victoria and you submit to the exclusive jurisdiction of the courts of Victoria.

This information helps our instructors keep training safe and support your participation. Personal and health information collected through this form is used for safety, participation, administrative, and legal purposes and will be handled confidentially by QCC staff.

Before training with us, please consider whether any of the following may be relevant to your participation: recent or ongoing injury, surgery or rehabilitation; heart, blood pressure, or cardiovascular conditions; history of concussion or neurological conditions; epilepsy; diabetes; pregnancy; or medical, physical, psychological, or accessibility condition relevant to your safe participation. Participants must not train while knowingly affected by alcohol, illicit drugs, or any condition that may place themselves or others at unreasonable risk.

If at any point during training you become aware of a condition, injury, or impairment that may affect your health or safe participation, you must stop training immediately and seek medical advice.

If you'd prefer to speak with an instructor privately before your first session rather than writing anything here, that's completely fine.

4. Understanding the Risks

About the Activities

You will be participating in combat sports training and group classes offered by QCC. Depending on the session, this may include boxing, Muay Thai, wrestling, MMA, and related fitness and conditioning activities. Specific activities vary by class and instructor. Training methods may include pad work, bag work, partner drills, grappling, ground work, conditioning, and supervised sparring where applicable.

Taking part in these activities carries inherent risk.

Participation may expose you to:

- Contact and physical impact with other participants in group settings - Participation in close-contact training environments may involve exposure to common illnesses or minor skin conditions. - Injury arising from your fitness level, fatigue, or overexertion - Injury from strenuous physical exercise - Injury related to your technique, experience level, or pre-existing conditions - Risks associated with equipment provided for training - Consequences of not following instructor guidance

These risks may result in personal injury, psychological or emotional harm, illness, property damage, or in serious cases, death.

I acknowledge that combat sports and martial arts activities involve inherent and obvious risks that cannot be completely eliminated, even where reasonable care is taken. I voluntarily choose to participate and accept those risks as my own.

You are never required to perform any activity you are not comfortable with. You may stop participating at any time.

5. Exclusion of Liability

The Australian Consumer Law (Cth) provides statutory guarantees in relation to the supply of services. Because these are recreational services, QCC may ask you to agree to limit those statutory rights, as permitted by law.

To the fullest extent permitted by law, QCC — including its principals, instructors, coaches, volunteers, contractors, subcontractors, and agents — excludes liability for personal injury, illness, psychological injury, death, property damage, or other loss arising from or connected with your participation in QCC activities, except where liability cannot lawfully be excluded, including where caused by reckless conduct or gross negligence.

You remain responsible for your own conduct and decisions during participation and agree to follow instructor directions, safety procedures, and equipment requirements at all times.

If any part of this waiver is found to be invalid or unenforceable, the remaining parts will continue to apply.

6. Victorian Statutory Waiver

Important Notice — Australian Consumer Law & Fair Trading Act 2012 (Vic)

If you sign this form, you will be agreeing that your rights to sue Queer Combat Collective under the Australian Consumer Law and Fair Trading Act 2012 are excluded, restricted or modified in the way set out in this form, if you are killed or injured because the services provided were not in accordance with the statutory guarantees outlined below.

Under the Australian Consumer Law (Victoria), several statutory guarantees apply to the supply of certain goods and services. These guarantees mean that the supplier named on this form is required to ensure that the recreational services it supplies to you—

- are rendered with due care and skill; - are reasonably fit for any purpose which you, either expressly or by implication, make known to the supplier; and - might reasonably be expected to achieve any result you have made known to the supplier.

Under section 22 of the Australian Consumer Law and Fair Trading Act 2012, the exclusion of these statutory guarantees is brought to your attention by this form.

NOTE: The change to your rights, as set out in this form, does not apply if your death or injury is due to gross negligence on the supplier's part. Gross negligence, in relation to an act or omission, means doing the act or omitting to do an act with reckless disregard, with or without consciousness, for the consequences of the act or omission. See regulation 5 of the Australian Consumer Law and Fair Trading Regulations 2012 and section 22(3)(b) of the Australian Consumer Law and Fair Trading Act 2012.

7. Community Standards

QCC is a welcoming and inclusive combat sports community.

As a participant you commit to:

- Treating all participants, instructors, and staff with dignity and respect, regardless of gender, gender identity, sexuality, race, religion, ability, or background - Honouring people's preferred names and pronouns when shared with you - Speaking openly with instructors about your comfort level, physical condition, and wellbeing - Avoiding discriminatory, harassing, or intimidating conduct or language of any kind - Following instructor guidance and observing all safety procedures - Reporting any injury — to yourself or another participant — to an instructor immediately

QCC may refuse or end participation where necessary for safety, wellbeing, or conduct reasons.

Community Guidelines

8. Photography & Media

Training sessions and events may be photographed or recorded for use in QCC's social media, community, and promotional content. Please indicate your preference below.

If you select 'do not consent', please let your instructor know at the start of each session. QCC will take reasonable steps to ensure you are not identifiable in any published content.

However, QCC cannot guarantee that you will not appear incidentally in background footage or photographs taken during group activities or public events.

9. Emergency Medical Authorisation

If you are injured or become unwell during a QCC activity and are unable to communicate your wishes, you authorise QCC instructors and event organisers to contact emergency medical services on your behalf. You acknowledge that any costs associated with emergency medical care are your responsibility. Participants are responsible for arranging their own medical, health, and personal insurance where desired.
