INFORMED CONSENT FOR RESEARCH
We invite you to take part in a research study. Please take as much time as you need to read the consent form. You may want to discuss it with your family, friends, or your personal doctor. If you find any of the language difficult to understand, please ask questions. If you decide to participate, you will be asked to sign this form. A copy of the signed form will be provided to you for your records.
PURPOSE: The purpose of this study is investigate if personality relates to mood. You are invited as a possible participant because you are a student.
PROCEDURES: If you decide to take part, this is what will happen: you will be asked to answer some questions about yourself.
RISKS AND DISCOMFORTS: Possible risks and discomforts you could experience during this study include slight discomfort posed by the questions about yourself. You can opt out of any questions or procedures that make you uncomfortable or end the experiment at any time without penalty.
BENEFITS: There are no direct benefits to you from taking part in this study. However, your participation in this study may help us learn about people.
PRIVACY/CONFIDENTIALITY: This study is anonymous, as we are not asking about your identity. Your responses, which are also called “data’, will retained indefinitely as they are not identifiable, thus cannot be linked to you in anyway. Your data and/or specimens collected as part of this research will be used or distributed for future research studies without your additional informed consent. Again, no information that identifies you (such as your name) will be taken or associated with the data, accordingly they can be shared with others or used in future research studies. The University of Southern California’s Institutional Review Board (IRB) and Human Subject’s Protections Program (HSPP) may review the data.
ALTERNATIVES: An alternative would be to not participate in this study.
PAYMENTS/COMPENSATION: You will not be compensated for your participation in this research.
VOLUNTARY PARTICIPATION: It is your choice whether to participate. If you choose to participate, you may change your mind and leave the study at any time. If you decide not to participate, or choose to end your participation in this study, you will not be penalized or lose any benefits that you are otherwise entitled to.
