Healthwatch Essex Survey Children with Asthma in North East Essex

On behalf of the NHS Suffolk and North East Essex Integrated Care Board (SNEE ICB), Healthwatch Essex are launching a project focusing on asthma in children and young people. As part of our project, we have designed a survey to learn about the experiences of both you and your child when using local NHS and community sector services for help with the condition. Please help us by taking a few minutes to complete the survey below, and help us make a real, positive change. Your voice is important! For further information, or to share your story, please contact the project lead Luke at Healthwatch Essex at [email protected] or call 07483 329658‬. We can also send you this survey in a different format if required. All participants will be kept anonymous and any information you provide will be strictly confidential. Healthwatch Essex may use quotations from this survey in our final report but will be attributed to an anonymous resident from one of the locations below (please select). Any additional information provided that could potentially identify you will be removed or modified before submission of the report. For more information on how we use and share information from the public please see our Privacy Policy or contact us by email at [email protected] or call 0300 500 1895.

7. Has your child visited Colchester or Clacton & District hospital at any point as a result of having asthma?

If yes, what was the reason for attending hospital?

e.g. Referral from your GP for diagnosis, or an emergency visit, etc.

Please give as much information as you feel comfortable sharing.

8. How is the asthma awareness and support at their school? Could more be done?

We would love to give you the opportunity to share your experiences in more detail. If you would be interested in taking part in a one-to-one interview, your experiences would really help us create change for the better. This would be a relaxed conversation in a comfortable environment (over the phone, online or in-person) where you can freely share your views and experiences in a private and confidential setting. This can be arranged at a convenient time to suit your availability.
If you are happy to be contacted, please leave your name, telephone number or email address below. All participants will be kept anonymous and any information you provide is strictly confidential. This is completely optional.
