Rock concert volume satisfaction survey

This survey is meant for people who attended at least one rock concert (or other amplified music concert, such as pop with amplified instruments) in the past 10 years.
Its objective is to understand the tolerance of audience members with various profiles toward concert loudness, and see if some volume adjustment would be preferable.
Please share this survey with other people who you know also attend rock concerts and are in age to fill this form. If you accompany children to concerts, reply to children-specific questions yourself instead.
Note that this is not a professional survey, so please understand that it may not fit all the industry's standard.
The survey takes about 5 minutes to fill (possibly more if you add free-form text answers, or if you give feedback on diverse concerts/bands with very different experiences).

Note about dealing with multiple concerts/bands
This survey was originally written for the amplified sections of concert "Sonic Symphony" and band "Crush40", so options in the list of concerts and locations are mainly related to them. Later, we added more band names mentioned by various people, in an attempt to make the survey more generic, but we couldn't cater for everyone, so feel free to specify other bands/locations in the Specify other fields.
We understand that people have different experiences across different shows. In this case, we recommend filling the form for the group of shows that you think was the most representative of your overall experience. Then, if you have time, you can fill the form again for every additional group of shows that gave you a similar experience regarding sound volume (e.g. all bands you think are Too loud together, then another concert you found Adequate) and room size. In this case, you will have to fill the common sections (Profile, General questions and Future) the same way each time. This is due to a limitation in form creation, and we apologize for the inconvenience.
