This scholarship program is for high school/college students ONLY.
Application deadline: September 20, 2025.
Number of Scholarships 1 : Up to 15 scholarships are

Available - ($500, $750, or $1000) each.

Note: Please upload all required documents at the end of the form

Select only one of the following scholarship types:

Academic Achievement Scholarship

Eligibility Criteria: 1- GPA of 3.5 or higher

2- Three recommendation Letters. One

from a teacher, and two from a

community leader, or employer (if

applicable).

3- A copy of the applicant’s college

transcript, report card, ACT/SAT (as

applicable), or high school diploma.

4- Resident of Illinois.

5- Non-recipient of MF Scholarship in the

past 3 years.

Financial-Based Scholarship

Eligibility Criteria:

1- GPA of 3.0 or higher

2- Three recommendation Letters from a

teacher, a community leader, or an

employer (if applicable)

3- A copy of the applicant’s college

transcript, report card, ACT/SAT (as

applicable), or high school diploma.

4- Proof of financial need such as a copy of

income tax papers, W2, etc.

5- Resident of Illinois

6- Non-recipient of MF Scholarship in the

past 3 years.
