I understand that any bodily reactions or occurrences post-tattooing are my sole responsibility and choice, and the artist cannot be held responsible for any unfavorable outcomes. In the event of uncertainty during or after the process, I commit to consulting either the artist or a professional. 

I verify that the information provided in this form is truthful to the best of my knowledge. I acknowledge that deposits are non-refundable, and failure to reschedule within 48 hours, cancellations, or no-shows will result in the forfeiture of the deposit. 

Finally, I am aware that the artist reserves the right to decline me as a client based on any areas of concern or if I am deemed unsuitable for tattooing.
