I, the undersigned, hereby confirm and certify that all documents and information submitted in connection with this application form are true, accurate, and complete and include all relevant details and matters that I am obligated to disclose to CARE ACADEMY (hereinafter referred to as “CARE”). I further agree to comply with and abide by CARE's decision concerning this application.

Consent for Use and Disclosure of Personal Data

In accordance with the Personal Data Protection Act (PDPA), I hereby provide my consent for CARE to: Share any information provided by me with relevant agencies for purposes related to this training application, the administration and provision of services or schemes, and for data analysis, evaluation, or policy formulation. CARE shall ensure that such information is anonymized and does not identify me as a specific individual unless otherwise required by law.

Capture and use photographs taken during the training programme(s) for use solely in CARE’s printed, text, and other visual content, such as course brochures, corporate website, social media platforms, banners, and posters. These photographs will not be circulated to third parties for commercial purposes and may be used as reference material through testimonials or similar uses.

Limitation of Liability

I acknowledge and agree that while CARE shall take reasonable precautions as required under any applicable laws to ensure my safety during training activities, CARE shall not be held liable for: Any personal injury, loss, or damage of any kind that I or other participants may sustain during the registered program(s) or at the training venue, except where caused by CARE's willful misconduct.

Any default or delay in performance caused by events beyond CARE’s reasonable control, including but not limited to acts of God, war, strikes, lockouts, industrial actions, fires, floods, droughts, tempests, or other unforeseen circumstances.
