Before camp starts, we want to get to know your child better. This form will gather information about your child, information about you, emergency contact information, pickup contact information, photo release, liability waiver, and general studio policies.

Are there any allergies that we need to know about? On the last day of camp, we celebrate with an ice cream party. Please let us know if there are any food allergies that we need to be aware of.
Are there any medications that you will be sending your child with? If medication is being sent with your child, please provide clear written instructions for dispensing and in the original container.
Is there anything we need to know to help make your child's experience better? i.e., past/current injuries or learning accommodations.
