Consent Form and Release for Little Music Makers

Joy Morin considers her teaching practice part of her personal research into the process of music learning and she is active as a pedagogical presenter, writer, and teacher-of-teachers. As part of her professional activities, she and her audience and readers may benefit from anecdotes, photos, videos or other images of your child engaged in music activities in class and demonstrations, and any you might submit to Ms Morin. Consent is required from each child’s parents for these purposes. Please review the consent form below and submit with your child’s registration for Little Music Makers.

I agree and grant consent to Joy Morin to use for pedagogical purposes, anecdotes about my child and their music activities, images, photos, or videos taken in Little Music Makers classes or elsewhere, including other media developed in the future and any print material that might be generated by your child in relation to music class activities. Pedagogical purposes include, but are not limited to, conferences/workshop presentations, online and print materials for teachers, and other such professional activities. To protect your child’s privacy, your child will never be identified by first and last names, but rather, either by first name and age, first initial, or no name at all. I represent that I am (we are) at least eighteen years of age, the parent(s) or guardian(s) of the child I am enrolling and am fully competent to sign this Consent.
