I hereby for myself and/or my child waive all rights and claims for damages I/we may have against “FLARE DANCE STUDIO” for any and all injuries received by myself and/or my child in connection with this studio for the year 2025-2026. A FDS t-shirt will be needed for the Christmas Show Fundraiser. There will be a cost of approximately $100 plus tax for the year end recital costume per class taken if participating in the show. RECITAL IS OPTIONAL.
