Hair Donation Application Form

💜 Cancer CARE Bangladesh (CCB) — Hair Donation Guidelines ━━━━━━━━━━━━━━━━━━━━━ ⏩ চুল দান করার জন্য নিচের নির্দেশনাগুলো মনোযোগ দিয়ে পড়ুন এবং নিয়মগুলো পালন করুন। ──── ✂️ চুল কাটার নিয়ম ──── ① এই ফর্মটি চুল কাটার আগেই পূরণ করতে হবে। ② চুল কমপক্ষে ১২–১৫ ইঞ্চি হতে হবে (ছেলে ও মেয়ে উভয়ের জন্য)। ③ চুল কাটার আগে শ্যাম্পু দিয়ে চুল ধুয়ে সম্পূর্ণ শুকিয়ে নিন। ④ বেনি করে বা রাবার ব্যান্ড দিয়ে ভালোভাবে বেঁধে তারপর কাটুন। বিঃ দ্রঃ ন্যাড়া করতে চাইলে আমাদের অভিজ্ঞ ভলান্টিয়ারের সাহায্য নিতে হবে চুল কাটার আগেই। ⑤ চুল কাটার পরে Air-Tight Zip Lock ব্যাগ অথবা পলি ব্যাগে ভালোভাবে প্যাক করুন। ⑥ কুরিয়ারে পাঠালে প্যাকেটের উপর মার্কার পেন দিয়ে ডোনার এর নাম-ঠিকানা লিখুন অথবা Printed Copy সংযুক্ত করে দিন। ──── 📲 যোগাযোগ ──── ফর্ম পূরণ করার পর আমাদের Official Facebook Page বা Official WhatsApp 01580919326-এ জানান। → আপনার এলাকায় অথবা আশেপাশে ভলান্টিয়ার থাকলে তারা সরাসরি চুল সংগ্রহ করবে। → না থাকলে Sundarban Courier বা অন্য কুরিয়ারের মাধ্যমে আমাদের অফিসের ঠিকানায় পাঠিতে হবে। ──── 📸 ছবি ও ভিডিও ────

💜 Cancer CARE Bangladesh (CCB) - Hair Donation Guidelines⏩

Please read the following instructions carefully before donating your hair and follow them accordingly.

──✂️ Hair Cutting Guidelines ──

① This form must be completed before cutting your hair.

② Hair must be at least 12–15 inches long (for both men and women).

③ Wash your hair with shampoo and let it dry completely before cutting.

④ Tie your hair securely with a braid or rubber bands before cutting.

📌 Note: If you wish to donate by shaving your head, you must contact us and receive guidance from our experienced volunteers before cutting your hair.

⑤ After cutting, place the hair in an air-tight zip lock bag or plastic bag and pack it properly.

⑥ If you are sending the hair through a courier service, please write the donor's name and address on the package using a marker pen or attach a printed copy containing the donor information.

→ If our volunteers are available in or near your area, they may collect your hair donation directly.

→ Otherwise, you will need to send your donation to our office through Sundarban Courier Service or any other courier service.

Please upload photos of your hair before cutting it through this form. Cut your hair only after receiving confirmation from us.

After cutting your hair, please take and keep both before and after photos and send them to our WhatsApp number or Official Facebook Page.

You are also welcome to record the hair-cutting moment as a video — it can inspire many others to support cancer patients through hair donation.

We never accept money from hair donors or cancer patients for any of our services.

All wigs and services are provided completely free of charge to cancer patients. Updates regarding supported patients are regularly shared on our Official Facebook Page.

Cancer CARE Bangladesh (CCB) is the only organization in Bangladesh currently providing free Original Human Hair Wigs to cancer patients.

Please do not hand over your donated hair to unknown individuals or unofficial organizations.

If anyone contacts you from a personal or Fake ID, unofficial page, or unknown number regarding hair collection or financial transactions, please be cautious and report it to us immediately.
