Participant Waiver & Release

By participating in the LWM Kickball Classic, I understand that kickball and related event activities involve physical activity and risks including falls, collisions, being struck by a ball, and other injuries. I voluntarily choose to participate and accept the ordinary risks associated with these activities.

To the fullest extent permitted by law, I release and hold harmless Living Word Ministries and its pastors, staff, volunteers, event organizers, officials, and agents from claims arising from my voluntary participation in the event, except to the extent a claim cannot legally be waived. I agree to follow event rules, officials’ instructions, and reasonable safety directions.

I authorize event personnel to seek emergency medical assistance for me if reasonably necessary. I understand that I am responsible for my own medical expenses and insurance coverage.

Participants under 18: A parent or legal guardian must complete the minor participant consent/release before the player may participate. LWM may require a separate parent/guardian signature before play.
