• Incomplete or approximate information may make it more difficult to identify a particular drop-off or item.

• This form is intended for merchandise dropped off approximately six months or more ago.

• Individual-item inquiries are limited to items with an estimated quick-sell value of $25 or more.

• Submitting multiple inquiries about the same merchandise will not speed up the research process.
