Trauma & Stress Check-in

This check-in is based on a widely used questionnaire (PCL-5) that helps explore how past stressful or overwhelming experiences may still be affecting you.

It reflects how you’ve been feeling over the past month.

This check-in is designed for self-reflection and is not a diagnostic tool.

You may keep a specific experience in mind as you respond, or answer based on what feels most relevant to you. You can go at your own pace—skip any question that doesn’t feel okay to answer.

Below is a list of problems that people sometimes have in response to very stressful experiences.

Please read each one carefully and indicate how much you have been bothered by that problem in the past month.
