Mentor Match is designed to connect Nurse Practitioners with experienced NPs who are willing to share their knowledge, experience and practical guidance, and who would also like to connect with other NPs in their field or locally.

Complete this application to be considered for inclusion in the NP Mentor Match network.

Your application will be reviewed and, if accepted, your mentor profile will be added to the NP Practice Fundamentals website individually.

By submitting this application, you confirm that you have read and agree to these Terms & Conditions and consent to the use of the information you provide for the purposes of the Mentor Match program.

NP Mentor Match — Mentor Terms & Conditions 1. About Mentor Match

NP Mentor Match is a networking and mentoring initiative operated by NP Practice Fundamentals.

The purpose of Mentor Match is to facilitate connections between Nurse Practitioners, NP students and other eligible participants who are seeking professional guidance, experience-sharing and networking opportunities.

NP Practice Fundamentals does not provide or guarantee formal mentoring, supervision, employment, recruitment or clinical placement through Mentor Match.

2. Mentor eligibility

By applying to become a mentor, you confirm that:

- you are a Nurse Practitioner or otherwise meet the eligibility requirements specified by NP Practice Fundamentals; - the information you provide in your application is accurate and current; -you have the experience and professional knowledge relevant to the mentoring areas you nominate; - you will act professionally and respectfully when communicating with other Mentor Match participants.

NP Practice Fundamentals may decline or remove a mentor application or profile where appropriate.

3. Mentor profile and publication

If accepted into the Mentor Match network, you consent to NP Practice Fundamentals publishing the information you have identified for inclusion in your mentor profile on the NP Practice Fundamentals website.

This may include information such as:

- your name;
- professional role; - state or location; - nursing and/or Nurse Practitioner experience; - specialty or professional interests; - areas in which you are willing to provide mentoring; - preferred mentoring format; - professional biography or introduction; - other information you voluntarily provide for your profile.

Only information intended for publication as part of your Mentor Match profile will be displayed publicly.

4. Contact and mentor connections

By joining Mentor Match, you understand that prospective mentees may use the information provided in your public profile to determine whether they would like to contact or connect with you.

NP Practice Fundamentals may facilitate an initial introduction or provide a method for participants to connect.

Once a connection has been made, communications and mentoring arrangements are between the mentor and mentee.

You are responsible for deciding whether you wish to accept a mentoring request and for establishing appropriate boundaries and arrangements.

5. Nature of mentoring

Mentor Match is intended for professional networking, knowledge sharing and mentoring.

Mentoring may include discussion of:

- career development; - transition into private practice; - professional experience; - clinical career pathways; - professional development; - practice establishment;
- specialty-specific experience; - general professional guidance.

Mentor Match is not a substitute for:

- formal clinical supervision; - mandatory professional supervision; - employer-based supervision; - university supervision; - professional or legal advice; -individualised legal, financial or accounting advice; -emergency or clinical consultation.

Mentors should not represent themselves as providing formal supervision or professional services through Mentor Match unless separately authorised and appropriately arranged.

6. Professional conduct

Mentors agree to communicate with mentees professionally and respectfully.

Mentors should maintain appropriate professional boundaries and comply with any applicable professional, workplace, regulatory and privacy obligations.

Mentors should not request or encourage mentees to disclose confidential or identifiable patient information.

7. No guarantee of outcomes

NP Practice Fundamentals does not guarantee:

- that a mentor will receive a mentee; - that a suitable mentor/mentee match will be available; - the frequency or duration of mentoring connections; - any particular professional, educational or career outcome.
- that Mentors are appropriately registered under AHPRA

Participation in Mentor Match is voluntary.

8. Responsibility for mentoring relationships

NP Practice Fundamentals facilitates the Mentor Match network but is not a party to individual mentoring relationships.

Mentors and mentees are responsible for assessing whether a mentoring relationship is appropriate for their individual circumstances.

NP Practice Fundamentals does not independently verify or guarantee the suitability, qualifications, experience, advice or conduct of another participant beyond any screening process it may undertake.

9. Privacy and personal information

Personal information collected through the Mentor Match application will be handled in accordance with the NP Practice Fundamentals Privacy Policy.

By submitting the application, you consent to NP Practice Fundamentals collecting and using your information for the administration and operation of Mentor Match, including creating and displaying your mentor profile where applicable.

You should only provide information that you are comfortable having used for these purposes.

10. Keeping information up to date

Mentors are responsible for notifying NP Practice Fundamentals if information contained in their mentor profile becomes inaccurate or needs to be updated.

You may also request changes to or removal of your mentor profile by contacting NP Practice Fundamentals.

11. Removal from Mentor Match

You may request removal from the Mentor Match network at any time.

NP Practice Fundamentals may also remove or suspend a mentor profile where it considers this necessary, including where there are concerns regarding accuracy of information, professional conduct, inappropriate use of the network or other circumstances affecting the integrity or safety of the program.

12. Changes to Mentor Match

NP Practice Fundamentals may modify, suspend or discontinue Mentor Match or amend these Terms & Conditions from time to time.

Where appropriate, participants will be notified of significant changes.

13. Acceptance

By submitting this Mentor Match application, you confirm that:

- you have read and understood these Mentor Terms & Conditions; - the information you have provided is accurate to the best of your knowledge; - you consent to the collection and use of your information for Mentor Match; - you consent to your nominated profile information being displayed on the NP Practice Fundamentals website if your application is accepted; and - you agree to participate in Mentor Match in accordance with these Terms & Conditions.
