Registration Form

Our eight-week school-based racquet sports program helps children develop coordination, confidence, teamwork and fundamental movement skills through fun, engaging activities.

Each program includes eight 40-minute sessions led by our instructors. All equipment is provided.

Registration fee: $260 + HST ($293.80 total).

Spaces are limited to 16 participants per group. Please complete the registration form and payment to secure your child's place.

SECTION 3 — PARENT/GUARDIAN AGREEMENTS

Participation and Safety
Racquet Play provides supervised, age-appropriate racquet sports instruction. Activities involve physical movement and may include risks such as slips, falls, collisions or accidental contact with equipment.

Instructors will establish and communicate safety expectations. Participants are expected to follow instructions, use equipment appropriately and demonstrate respectful behaviour toward others.

14. Participation Acknowledgment

I am the parent or legal guardian of the child being registered. I authorize my child to participate in the Racquet Play program, acknowledge the ordinary risks associated with physical activity, and agree to provide accurate information relevant to my child's safe participation.

15. Emergency Assistance Authorization

If my child requires urgent assistance, I authorize Racquet Play personnel to contact emergency services and provide relevant information to responders. I understand that Racquet Play will make reasonable efforts to contact me or the designated emergency contact promptly.

16. Do you consent to Racquet Play using identifiable photographs or videos of your child for promotional purposes?

Yes, I consent.

17. Refund Policy Acknowledgment

Cancellation & Refund Policy All Racquet Play registrations are final. No refunds, credits, or transfers will be issued for voluntary withdrawals, missed sessions, scheduling conflicts, illness, or other personal circumstances, except where required by applicable law. If Racquet Play or the school/school board cancels a program before it begins, a full refund will be issued. If a program is cancelled after it has started and cannot be completed, a prorated refund will be provided for the sessions not delivered. Racquet Play reserves the right to reschedule sessions due to school closures, inclement weather, or circumstances beyond its control. Families will be notified of any schedule changes. If a cancelled session cannot be rescheduled, a prorated refund will be issued for that session. By completing registration, you acknowledge that you have read and agree to this Cancellation & Refund Policy.

18. Privacy Acknowledgment

Racquet Play respects your family's privacy. We collect student, parent/guardian, emergency contact, and relevant health information solely to manage registrations, deliver programs, communicate with families, and ensure participant safety.

Information is shared only with authorized instructors, school personnel, or service providers when necessary. Payments are securely processed through Stripe. Personal information is protected through reasonable safeguards and retained only as long as necessary for operational or legal purposes. Information may be processed outside Canada.

Photographs and videos are used for promotional purposes only with separate parental consent.
