You are invited to take a confidential survey about your experiences in the military. The purpose of this survey is to seek input from service women, women veterans, military-connected community members, and coalition partners we advocate for and meet the needs of service women and women veterans.
SWAN aims to accurately understand the needs and any gaps in services so that we can advocate for care. Findings may be used to inform SWAN’s policy positions, advocacy efforts, educational materials, and engagement with policymakers.
This survey is not a formal reporting mechanism and should not be used to report an emergency, request medical assistance, initiate a VA or DoD complaint, or report sexual assault or sexual harassment.
Participation in this survey is voluntary. You may skip any question or stop the survey at any time.
Individual responses will not be shared with the Department of Veterans Affairs, the Department of Defense, military commands, healthcare providers, or other government agencies.
Survey results may be reported publicly or shared with policymakers only in aggregate or de-identified form. SWAN will not publish information that could reasonably identify an individual respondent.
Please do not include specific facility staff names, claim numbers, medical record numbers, or other identifying information in your written responses.
IP addresses are not collected.
