This questionnaire is designed to identify individuals for whom certain physical activities may be inappropriate, or who should seek medical advice before participating. Please answer all questions honestly.

If under 18 years of age, please complete this form in the company of a parent / guardian.

This form must be completed by all athletes before participation in the EKA York competition. The purpose of this form is to support athlete safety, ensure appropriate medical awareness, meet UK duty of care expectations, and support insurance and safeguarding requirements. All information will be handled confidentially and stored in line with UK GDPR.

Athletes are expected to answer all questions honestly. If you are unsure about any question, you should speak to the event organiser or first aider before signing.

Privacy Notice We are collecting this Physical Activity Readiness Questionnaire (PAR-Q) to assess your suitability for physical activity and ensure your safety during our programmes. Your health data: This form collects special category personal data about your health. We process this information under Article 9(2)(h) of UK GDPR (health and social care purposes) and our legitimate interests in providing safe fitness services. Data storage: Your responses will be securely stored using Google Forms (Google LLC). While Google employs industry-standard encryption and security measures, please note this is a cloud-based service and data may be stored outside the UK under appropriate safeguards. Your information: Your PAR-Q responses will only be accessed by authorised EKA staff for fitness assessment and safety purposes. We will retain this data in accordance with our standard retention policy. Your rights: You have rights to access, correct, delete, and object to processing of your data.
