I. Waiver of Liability
I, the undersigned, hereby give permission for my child to participate in the NEJA Nitzanim Summer Program at New England Jewish Academy. I understand that while every effort will be made to provide a safe and secure environment, participation in any program involves inherent risk. I agree to hold harmless NEJA, its employees, agents, and volunteers from any claims, damages, or liabilities arising from injury, accident, or illness incurred during my child's participation, unless caused by gross negligence or willful misconduct.
II. Medical Authorization
In the event of an emergency where I cannot be reached, I authorize NEJA staff to obtain emergency medical care for my child. I understand that every effort will be made to contact me prior to such action. I accept full responsibility for any charges related to such medical care. My child’s health information, allergies, and emergency contacts have been submitted or will be submitted on the appropriate forms.
Please list all authorized pick up persons, their full names, relationship to student, and their phone number (other than parents).
Your consent covers only the document(s) for which you are presently signing. You understand and agree that your electronic signature on any document presented that requires a signature shall be legally binding and such transaction shall be considered authorized by you. Checking the box creates an electronic signature that has the same legal force and effect as a handwritten signature.
