This form is for people who have already been accepted into a Love In Action or Love City Arts Academy program. Complete only the agreements for your approved role. Do not use this form to apply for a new opportunity.
For students under 18: The parent or legal guardian completes this entire onboarding form on the student's behalf. The minor does not fill out or sign any part of the packet. Enter the student's information in participant fields, your own information in guardian fields, and sign each applicable agreement as the parent or legal guardian. Staff verifies guardian authority and all required signatures before onboarding is approved.
