If you are 18 or older, you may complete this yourself.
In exchange for participation at the [Identity Youth / Identity Church] Beach Day held at [Lakeview Beach in Lorain, Ohio] on Thursday, July 9, 2026, I, the undersigned, hereby agree on behalf of myself or my child as follows:
1. Liability Release & Discharge
I release and forever discharge Identity Church, Dylan Wymer, and their affiliates, successors, assigns, officers, employees, volunteers, representatives, and agents (collectively, the "Releasees") from any and all claims, demands, causes of action, or liabilities of any kind, known or unknown, arising out of or relating to any injury, loss, or damage to person or property that may be sustained as a result of participation in this event, including but not limited to transportation provided by the leaders.
2. Swimming and Open Water Activity Acknowledgement
I acknowledge that this event involves swimming, wading, and other beach activities in an open-water environment (lake).
- I certify that my child is a proficient swimmer capable of navigating open water conditions.
- I understand that open water presents inherent risks, including currents, sudden depth changes, and changing weather conditions.
- I agree that my child will follow all lifeguard instructions, designated swimming boundaries, and safety rules established by the youth leaders.
3. Sun and Environmental Exposure
I acknowledge that this is a full-day, outdoor event. I understand that parents/guardians are responsible for providing sunscreen and appropriate beach attire for their child. The Releasees are not liable for environmental injuries such as severe sunburn, heat exhaustion, or dehydration.
4. Indemnification and Binding Agreement
I agree to indemnify and hold harmless the Releasees from any and all claims, damages, liabilities, and expenses, including legal fees, arising out of or related to participation in the Beach Day. This waiver shall be binding upon myself and, if applicable, my child, and our respective heirs, legal representatives, and assigns.
5. Event Schedule & Transportation Permission
If signing on behalf of a participant under 18, I give permission for my son/daughter to attend the Beach Day on Thursday, July 9, 2026.
Drop-off: Participants will meet at Identity Church at 10:00 AM on Thursday. Dylan Wymer and associate leaders will provide transportation to the venue.
Pick-up: Estimated return time to the church is 10:00 PM on Thursday. Parents or guardians are responsible for pickup unless alternate arrangements are approved by Dylan Wymer.
By signing below, I confirm that I am either the participant (if age 18 or older) or the parent/legal guardian (if under 18) and that I agree to all terms stated above.
