Douglas County Animal Welfare Community Survey

We're in the early stages of building Umpqua Animal Society, a new nonprofit focused on animal welfare in Douglas County, Oregon. Before we decide what programs to launch, we want to hear directly from the community.

This survey takes about 4 minutes. Every question is optional, skip anything you're not comfortable answering. Your responses are completely anonymous unless you choose to leave your contact information at the end.There are no wrong answers. We genuinely want to know what the community needs.

This survey is anonymous. We do not collect your name, email, or any identifying information unless you voluntarily provide it in the final section. Responses are stored securely and used only for program planning purposes. We will never sell or share your information.
