WAIVER & RELEASE OF LIABILITY
In consideration of being permitted to participate in The Force Dance Academy workshop/clinic (the "Event"), I, the undersigned, on behalf of myself and/or my minor child, hereby agree to the following:
1. Assumption of Risk. I understand that participation in dance activities involves physical exertion and carries inherent risks of injury. I voluntarily assume all risks associated with participation in this Event.
2. Release of Liability. I hereby release, waive, and discharge Force Dance Academy, its owners, directors, instructors, employees, volunteers, and agents (collectively, "Released Parties") from any and all claims, damages, or liability arising out of or related to participation in this Event, including but not limited to personal injury, accident, illness, or property damage.
3. Medical Authorization. In the event of an emergency, I authorize Force Dance Academy staff to seek and consent to medical treatment on behalf of the participant. I acknowledge that I am responsible for all associated medical costs.
4. Photo & Video Release. I grant Force Dance Academy permission to photograph and/or record the participant during this Event and to use such images or footage for promotional and educational purposes.
5. Medical Conditions. I confirm that the participant is in good physical health and able to participate in dance activities. I have disclosed any known medical conditions, allergies, or injuries below.
