Summer Camp Scholarship Nomination

DUE APRIL 1, 2025
This form is intended for teachers and other community members to nominate students for a summer camp scholarship at the Wisconsin Humane Society. Summer camps are five days long and located at our Milwaukee, Racine, and Ozaukee campuses.
(If you are a parent that would like to apply for a scholarship for your own child, please fill out the scholarship application form here.)
Teachers/mentors may nominate more than one student - complete one form for each student you are nominating.
Once you submit this form, an automatic email will be sent to you AND the parent/guardian with a link to the scholarship application form that the family must complete and submit by April 15.
Nominations must be submitted by April 1 to allow for enough time for families to complete and submit the scholarship application by April 15.

We may contact you to help follow up with families if we do not receive their application.
Applicants will be notified whether they have received a scholarship by May 15 via email.
