Membership Cancellation Request Form – Yoga Tree

Thank you for submitting your cancellation request. Please note that this is only a submission and does not confirm cancellation until you receive a Cancellation Confirmation Number via email within 48 hours from submission. A minimum of 14 days' notice is required, and cancellations cannot be backdated. Please complete all fields accurately. We do not accept any phone or verbal cancellations.
Minimum 2 Weeks Notice Please complete this form to request the cancellation of your membership. We require a minimum of 2 weeks' notice before your charge date. If you need to cancel with less than 24 hours' notice, a $50 rush fee applies.

Keep Your Receipt: Your cancellation is not complete without an official email confirmation receipt. You are required to keep this as your only valid proof of cancellation.

Mat Storage Can Only Be Cancelled In Person After Retrieving Your Mat If you have a mat in storage, please pick it up in person on the same day you submit your cancellation request. Mat storage cancellations can only be completed once the mat has been retrieved and the storage has been cleared. Yoga Tree is not liable for any lost, stolen, or unattended items. Do not store any valuables. We are not responsible for lost or stolen items. For any questions, contact us directly. I acknowledge and understand that this is a cancellation request only and does not guarantee immediate termination of my membership. Once the request has been processed and cancelled successfully, an official Cancellation Confirmation Number will be issued within 48 hours (excluding holidays). Same-day processing only applies to members who requested and paid the express cancellation fee of $50. If you do not receive a confirmation email within this timeframe, please get in touch with us at ask@yogatree.ca. Your request is not final until you have received a Cancellation Confirmation Number.
