I, ____________________, hereby acknowledge that I am the owner of ___________________, and I understand that grooming and boarding involve certain risks, including but not limited to injury, illness, or damage to my pet. I agree to assume all risks associated with the grooming and boarding services provided by Hopeland Pet Lodge.
I hereby release, waive, and discharge Hopeland Pet Lodge, its employees, and agents from any and all claims, liabilities, or damages arising from any injury or harm that may occur to my pet during the grooming or boarding process, including any unforeseen reactions to grooming products or any incidents that may occur while my pet is being boarded.
In case of an emergency, I authorize Hopeland Pet Lodge to seek veterinary care for my pet if necessary, and I agree to be responsible for any associated costs.
I have read this waiver and fully understand its terms. I am signing this waiver voluntarily and with full knowledge of its significance.
