GLOWDEGA MEMBERSHIP APPLICATION

We offer three membership tiers that can be paid on a monthly/annual basis.

On average, our members save $850 each year! In addition to receiving deeply

discounted services, members also receive discounts on products, exclusive

events, and annual gifts. Memberships require a 6-month commitment (for

monthly members). Afterwards, they may be cancelled at any time. An early

termination fee of $150 is applied for cancellations inside of the 6-month

commitment window.

I understand that there is a 6-month minimum commitment for all monthly membership tiers and that if I wish to cancel before the 6-month minimum commitment is met, I will be charged a $150 early termination fee.

I understand that membership perks such as gifting services, additional discounts, and waived fees may only apply to select annual memberships.

I understand that monthly services are only valid for their billing duration otherwise they will expire. Unused monthly services may not be accumulated for future use or rolled over.

I understand that annual services are only valid for the 12-months following the initial purchase date. Unused services may not be accumulated for future use or rolled over into a new calendar year outside of the billing duration.

I understand that I cannot substitute membership services for other services that may be offered.

I understand that refunds will not be issued for unused memberships.
