Before we get you sparkled up, we just need you to read and sign this quick form so we’re all on the same page.

Please read and agree to the following:

• I understand my jewelry will permanently welded closed until I choose to have it removed with cutters.
• I know there’s a small risk of pinching, heat, or skin irritation during the process.
• I’m responsible for choosing my chain, metal type, and placement.
• I don’t have any medical conditions or metal allergies that would make this unsafe.
• I know I may have to remove the jewelry for medical procedures, certain jobs, or safety reasons (MRI, surgery, or other possible medical scans).
• I release CE - Permanent Jewelry, its staff, and event hosts from any responsibility if I have a reaction, injury, or other issue after today.
