We/I indemnify Goju Ryu Karate Centre, its instructors and assistants, against any accident, injury or loss caused in any way whatsoever or cost incurred on account of such injury or loss that may occur as result of participating in the The Jagger & Friends Gashuku. We know of no prior health issues or current injuries that might compromise participation in this event.
If I/my child is ill, I will notify the organisers and withdraw my entry.
I acknowledge that I will reserve a space once this form is submitted, and I will pay into the Goju Ryu Karate Centre account once I have received my invoice.
