Waiver And Release Of Liability Terms

These may arise from my own actions, the actions or negligence of others, or external circumstances beyond the control of the company. I acknowledge these risks and voluntarily assume them.

• My participation is entirely voluntary.

• I affirm that I am in good health, physically able, and not under the influence of alcohol, illegal drugs, or any substance that may impair my performance.

• I agree to strictly follow all event safety protocols, staff instructions, and equipment usage guidelines issued before or during the activity.

I, as participant and on behalf of my heirs and assigns, hereby release and discharge the company and its officers, employees, agents, and contractors from any liability for claims, demands, damages, or causes of action, including negligence arising from my participation.

I agree to indemnify and hold harmless the Released Parties against any damages or expenses including but not limited to legal fees, arising from any claims brought because of my actions or conduct during the activity.

• In case of injury or illness, I authorize the company to arrange necessary medical care at my expense.

• I agree to provide an Emergency Contact information within the given fields in this form to be used in the case of emergencies.

I grant the company the right to record and use photographs, audio, and video footage of me during the event for promotional, marketing, or archival purposes, without compensation.

• If any provision is found unenforceable, the remaining terms stay in effect.

• This agreement is governed by and construed under the laws of the Republic of the Philippines, and any dispute will be brought before Philippine courts.

I have fully read this Waiver and Safety Guidelines Agreement, understand it, and sign it voluntarily.
