Patient Online Registration

Terms and Conditions (T&Cs) for patient online registration outline the legal agreement between the healthcare provider and the patient, covering responsibilities, data usage, and limitations of the service. Key clauses typically include the following:

Core Patient Responsibilities

Accuracy of Information: Patients must agree to provide truthful, accurate, and complete information during registration and in all questionnaires. Providing false information may be a criminal offense and can lead to refusal of service or other legal action.
Personal Use and Age Requirement: The registration is generally for the patient's personal, non-commercial use only. Users typically must be 18 years of age or older to register and use the services.
Account Confidentiality: The patient is responsible for safeguarding their login credentials (username, password, OTP) and for all activities that occur under their account. They must not share their account details with anyone else.
Prompt Monitoring: Patients are responsible for monitoring electronic messages from the healthcare team via the portal, as they may contain important health information.
Emergency Disclaimer: Patients must acknowledge that the online service is not for medical emergencies and that in a serious case, immediate assistance from emergency personnel should be sought.

Healthcare Provider's Role and Limitations

Service Description: The T&Cs describe the specific services offered through the online portal (e.g., booking appointments, accessing records, messaging, getting health advice).
Medical Advice Disclaimer: Information on the portal should not be construed as a replacement for a relationship with a qualified healthcare professional or relied upon for diagnosis or a course of treatment.
Privacy and Data Security: A link to the separate Privacy Policy is included, which details how patient personal and medical data is collected, used, and secured. Patients acknowledge that while measures are taken, no system can guarantee absolute security.
Limitations of Liability: The provider limits its liability for any injury, loss, or damage resulting from the use of the portal, a failure to provide accurate information, or a failure to follow advice.
Right to Refuse/Terminate: The provider reserves the right to deny access, terminate accounts, or refuse service at its discretion, particularly in cases of a breach of the T&Cs (e.g., abusive behavior, providing false information).
Changes to Terms: The provider typically reserves the right to modify the T&Cs at any time, with continued use of the service signifying acceptance of the revised terms.

Consent and Authorization

Implied Consent: By accessing and using the portal and completing registration, the patient indicates their acceptance of the T&Cs and the associated privacy practices, creating a legally binding agreement.
Treatment Consent: The registration form may include an authorization for the medical provider to administer necessary treatments or procedures and to release medical information for insurance claims processing.
Proxy Access: Specific conditions are outlined for situations where a patient schedules an appointment or manages an account for a family member or friend, clarifying the responsibility of the account holder.
