Vendor Submission Form

This form is your submission to request to become a vendor for Kava Luv's Handmade Market. This event is held Saturday August 15th at 1335 W Elizabeth St, Suite 140, Fort Collins CO 80521.

Submitting this form does not grant you a definite spot. Vendors will be chosen based on products and services as we rotate vendors each event to avoid conflict.

If accepted you will receive an email or text within a month of the event date you are selected for with confirmation of the date approved.

Vendor Fee: We are currently offering zero fees for first time vendors.

Event Time: 1pm- 6pm

Set-up Time: 12:30pm

Tear-down: 6:30pm

If more time is needed for set-up or tear-down, please list in the comments and concerns box.

Tear-down is to begin at 6pm with 30 minutes to safely remove all items. All items must be packed up and safely out of the way of customers and staff by 6:30pm.

Release of Liability/Waiver Form/Market Policies: Please read the following policies; By checking the boxes, you agree to these polices.

I forever release Handmade Market, Kava Luv Social Lounge, and all other parties involved from any and all actions, claims, or demands that I, my assignees, heirs, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death, or property damage, related to (1) my participation in Handmade Market, (2) the negligence or other acts, whether direly connected to these activities or not, and however caused, by any Release, or (3) the condition of the premises where these activities over. I have read this document in its entirety and I freely and voluntarily assume all risks of such injuries and damages and notwithstanding such risks, I agree to participate in Kava Luv's Handmade Market.

I understand the No Refund policy. There are no refunds on vendor market fees.

I understand the No Rollover policy. The no rollover policy means that I may not rollover my market fees due to not being able to attend the market for any reason.

I understand the No Call, No Show policy, If you do not show up to the event, or leave early from the event, you will not be considered for future markets organized by Kava Luv Social Lounge. We cannot stress how important communication is for a successful event. If you are unable to attend, please give us a two week notice in order to remain on the vendor list for future events.

I understand that I must show up on time to the event & maintain proper communication if there is a delay in my arrival.

I understand that I can only sell the items that are listed in my application. For example, if your approved item is handmade candles, you cannot sell any other items. All items must be listed out on your application and approved ahead of time.

I understand my invoices must be paid on time, by the due date listed on the invoice, or my invoice will be canceled. Upon approval, you will receive and invoice from Kava Luv Social Lounge (this must be paid on time or invoice will be canceled and you will need to reapply).

I understand that MLM companies/Direct Sales/Large corporations/Franchises will not currently be approved to sell at Kava Luv's Handmade Market. For example MaryKay, Herbalife, Monat, etc. Additionally, we are not allowing booths that are religious or politically affiliated.

I understand that vendor insurance is recommended but not required. Kava Luv Social Lounge is expressly not responsible for lost or damaged products, items, merchandise, tents, tables, chairs, or any items within your set up.
