Waiver Information

Note: Woodvale operates under a set of policies, including the “Child and Youth Protection Policy”. This applies to all events, both on-site and off-site. Please note that although we strive to meet a high standard of care for all events, we cannot always control the environment for off-site events where meeting this standard may not always be possible.

Event Date: July 6, 2026  Event Time: 5:30 PM - 8:00 PM

Dear Parent: Woodvale Pentecostal Church has arranged activities at Britannia Beach on your behalf for your child(ren). No transportation arrangements will be available, you will be responsible for dropping off and picking up your student(s).

Details of the Activity: While every precaution is taken for the safety and good health, some activities carry with them the inherent risk of personal injury. Your permission is required allow your child(ren) to participate and attend. Please carefully read the following information and consent form. If you are in agreement, please provide consent in this form. PERMISSION: I give permission for my child/charge (“child”) to attend the Beach Night at specified location on the date indicated. I understand that my child is expected to follow all applicable rules given by their Pastor(s) and Leader(s) and directions provided by the park or adult volunteers. I understand that participation in the identified event is not a requirement for participation in Woodvale Pentecostal Church’s activities. I have read, understand, and discussed with my child that: • They are expected to respect each other, the beach, and the people they are surrounded by throughout this event. • Physical activity and beach activates may result in personal injuries and/or death and Woodvale Pentecostal Church will not be held responsible. • I have outlined all allergies my Child(ren) may have and all food provided will be eaten at own will and risk. • I recognize that by participating in this activity, as with any activity involving physical activity, my child may risk personal injury or permanent loss. I hereby attest and verify that I have been advised of the potential risks, that I have full knowledge of the risks involved in this activity, and that I assume any expenses that may be incurred in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expenses.

Consent for Activity Participation

I hereby consent to the participation of my/our child(ren) in this supervised activity.

I/we, the parents or guardians named below, authorize the Director or one of the (organization’s) Personnel to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.

I/we, named below, undertake and agree to indemnify and hold blameless Woodvale Pentecostal Church, its Pastors, its Directors and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Woodvale Pentecostal Church as well as of any medical treatment authorized by the supervising individuals representing Woodvale Pentecostal Church. This consent and authorization is effective only when participating in or travelling to events of the Woodvale Pentecostal Church.
