Liability & Participation Agreement

Assumption of Risk
I understand that participation in
dance sessions, rehearsals, workshops,
classes, camps, performances,
conditioning sessions, stretching, and
related activities involves physical
exertion and carries inherent risks.
I voluntarily choose to participate with
full knowledge of these risks.

Medical Condition Disclosure
1) I certify that I am physically able to
participate in dance activities. 2) I agree
to disclose any medical conditions,
injuries, allergies, physical limitations,
or medications that may affect my
participation. 3) I understand that it
is my responsibility to consult with
my physician before participating
if I have any medical concerns.

Release of Liability
To the fullest extent permitted by law, I
release and hold harmless the dance
studio/company, its owners, instructors,
assistants, volunteers, employees,
contractors, affiliates, and facility owners
from any liability, claims, demands,
damages, losses, or causes of action
arising from my participation, except
where prohibited by law or caused by
gross negligence or intentional
misconduct. This release applies to
personal injury, illness, property
damage, or other losses sustained
during participation or while on the
premises.

Emergency Medical Care
In the event of an emergency, I authorize
Split by Naéture, its affiliates, and
associates to seek emergency medical
treatment on my behalf if I cannot do
so myself. I understand that I am
financially responsible for any
medical expenses incurred.

Personal Responsibility
I agree to:
Follow all instructor directions. Use
proper technique to the best of my
ability. Participate safely and
responsibly. Notify the instructor
immediately if I experience pain,
dizziness, injury, or illness. Respect
fellow participants, instructors,
and studio property.

Conduct Policy
To help maintain a safe and welcoming
environment, participants agree to:
1)Treat everyone with kindness and
respect. 2)Refrain from bullying,
harassment, discrimination, or
disruptive behavior. 3) Wear
appropriate dance attire and footwear.
4) Arrive on time and prepared for
class. 5) Refrain from participating
while under the influence of alcohol
or illegal drugs. 6) Participants whose
behavior creates an unsafe or
disruptive environment will be
removed.

Photography & Media Release
Yes, I grant permission for photographs
and videos of myself (or my child) to be
used for promotional, educational,
website, and social media purposes
without compensation.

Personal Property
I understand that Split by Naéture, its
affiliates, and its associates are not
responsible for lost, stolen, or damaged
personal belongings.

Attendance & Refund Policy
I understand that:
1) Registration reserves my place
in the specified event. 2) Class and
camp ticket purchases are not
refundable. 3) Cancelled classes and
camps will be refunded.

Minor Participant
If the participant is under the age of 18,
I certify that I am the parent or legal
guardian and have the authority to
sign this agreement on the participant’s
behalf.

Agreement
I have carefully read this Liability and
Participation Agreement and fully
understand its contents. As the
Guardian, legal representative, of the
camp participant, I voluntarily agree to
all terms within this agreement, and I
acknowledge that I am giving up
certain legal rights by purchasing a ticket.
