To facilitate the selection of award recipients, it is important that the Honors & Awards (H&A) Committee have complete and accurate information. Please provide the requested information to the best of your ability. We will collect additional information from nominees as needed.
Nominations are reviewed by the H&A Committee beginning in early December and approved by the IS&T Board of Directors. The deadline for all awards is November 1 for each calendar year.

We require statement of nomination from the nominator (maximum 1000 words, either as text or an upload document). This statement should describe why the candidate is nominated. In addition, we require signed letters of support from 2 different recommenders on appropriate letterhead. The CV of the nominee is required for the assessment by the Honors and Awards committee who evaluates the nominations and provides their recommendations for the IS&T Board to approve. An incomplete packet disqualifies the nomination.
