The organization OneQuantum PH/Quantum Computing Society of the Philippines seeks to gather information from the respondents of this form. Data pertaining to the cognitive aspects of participants may be used as data for pedagogical researches on quantum education.

In compliance with the Data Privacy Act of 2012, the information provided by the participants are only collected, processed, stored and disposed by duly authorized QCSP members, exclusively for the aforementioned purposes and in identifying participants and potential organization members. Furthermore, storage of the collected data will only be kept for two (2) years upon collection.
