By signing in the box below, I (the Parent/Legal Guardian) hereby consent to the participation of my youth in this supervised activity. While every percaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury. I understand and accept these risks and agree that by allowing my youth (named above) to participate in those activities, they may be taking part in a recreational activity that presents the potential for personal injury. I, the Parent or Legal Guardian named above and signing below, authorize the Director or one of The Table CC's personnel or volunteers to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment, or procedures for the participant (youth named above). By signing below you agree to have read, understood, and agree with the above Consent Waiver statement.

Liability Waiver

I, the Parent or Legal Guardian named above and signing below, undertake and agree to indemnify and hold blameless The Table Commnunity Church, its Personnel, its Leaders, its Volunteers, and its Board, as well as the Totonto International Collegiate, its Personnel, its Leaders, its Volunteers, and its Board from and against any loss, damage, or injury suffered by the participant (youth named above) as a result of being part of the activities of The Table Community Church and the Toronto International Collegiate, as well as any medical treatment authorized by the supervising individuals representing The Table Community Church or Toronto International Collegiate. This consent and authorization is effective only when participating in or traveling to events of The Table Community Church and Toronto International Collegiate. By signing below you agree to have read, understood, and agree with the above Liability Waiver statement.

Photography & Video Release Waiver

By attending The Table Community Church or Toronto International Collegiate events and programs, you (the Parent named above and signing below) hereby grant permission to photographing and/or recording video of me and my youth (named above) during The Table Community Church ministry activities, programs, and events. I, the Parent or Legal Guardian, understand that these photos and videos may be used for ministry purposes including, but not limited to, promotional materials, social media, website content, presentations, publications, and other communications related to The Table Community Church and Toronto International Collegiate. I, the Parent or Legal Guardian, acknowledge that all photos and recordings are the property of The Table Community Church and Toronto International Collegiate and may be used without compensation. I waive any right to inspect or approve the finished product or the manner in which it is used. I release The Table Community Church and Toronto International Collegiate, its Personnel, its Volunteers, and its Representatives from any claims, demands, or liability arising from the use of these images or recordings. This authorization shall remain in effect unless revoked in writing. By signing below you agree to have read, understood, and agree with the above Photography & Video Release Waiver statement.
