PROFESSIONAL USERS - Annual approval form for regular customers

PARAMOSE RANGE OF DICHLOROMETHANE, METHEYLENE CHLORIDE, (DCM) PAINT REMOVERS

PROFESSIONAL USE is defined as the use of dichloromethane paint remover outside of a unit or workshop, where paint stripping normally takes place This declaration is to confirm that in accordance with European Regulations 455/2009/EC, the above products will only be supplied to/or

used by Industrial or Professional Users.

We require PROFESSIONAL USERS to complete this declaration and provide us with details of your HSE Certificate and Certificate No. and the Identity of the Company who issued your Certificate.

If the product is intended for professional use. the following applies:

1) If you are undertaking the work you must inform us that the persons using the product have undergone training from an approved HSE trainer, and have passed the online test

2) If you are selling on to a Professional User you need to keep records as in (1) above and obtain your own declaration from your customer.

INDUSTRIAL USE: To be used within an INDUSTRIAL ENVIRONMENT. The definition of Industrial is ‘Facilities where paint stripping takes place’. Examples would be units and workshops used for stripping articles with either dip tanks or brush applied systems. If this applies to you please complete the Industrial User Form.
