A working application for the year. Approximately 30–60 minutes if you write thoughtfully. The questions are designed to produce real thinking, not just information — so take them seriously even if you decide not to send the form.

Before you begin

A Few things to know before you start:

This is not a quick form. I designed it to do real work for you, regardless of whether we end up working together. Most people find that completing it brings more clarity than they expected. Set aside 30 to 60 minutes when you have quiet time. Don't try to do it on a phone in line somewhere.

Write honestly, not impressively. I'd rather read messy, true sentences than polished ones. The whole year ahead would be built on the truth you bring here, so it serves you to begin in that register.

Don't research your answers. If a question stumps you, that's information. Sit with it. Write what's actually true rather than what you think the right answer would be. There are no right answers to any of these.

You can save and come back. If the form supports drafts, use them. If it doesn't, draft your answers in a separate document first, then paste them in.

What happens after. Once you submit, I read the form carefully — usually within a few business days. If it seems like a fit, I'll write you back with a real letter (not a templated yes), and we'll go from there. If we're not the right match, I'll say that too, and I'll do my best to point you toward what might actually serve you.
