A valid SIN or work permit is required for work and may be requested prior to booking

I will complete my Worker Health & Safety Awareness in 4 Steps Course: https://www.labour.gov.on.ca/english/hs/elearn/worker/index.php and have hard copies available at all times when I am booked on set*

I will have available Proof of Residency documents (digital copies) available to submit to production

Utility bill, Notice of Assessment from last year, dollar figures edited out is acceptable.
