For parents and caregivers of children aged 4–17

This check-in is based on a widely used questionnaire (SDQ) that helps explore children’s emotional wellbeing, behaviour, and social experiences.

It reflects your observations of your child over the past 6 months.

This is not a diagnosis, but a way to better understand your child’s strengths and any areas where they may need additional support.

Every child develops differently, and behaviours can change over time and across situations.

There are no right or wrong answers—just respond based on what you’ve observed for the past 6 months.
