Contract

The person signing this contract is assumed to be the legal owner of the pet(s) listed and warrants that the information provided on the pet information sheet is true and correct. The Owner agrees to indemnify Blue Dog David for any liability caused by any inaccurate information listed on the pet information sheet. The owner guarantees that the pet(s) is fully vaccinated and in good health and has not harmed or shown aggressive or threatening behavior towards any person or other dog. The owner guarantees that the pet has not been exposed to Rabies, Parvovirus, or Distemper within a thirty (30) day period prior to any boarding or daycare both today and in future stays at Blue Dog David. Additionally, Owner disclaims any claim for a loss of pet as a result of contacting Parvovirus or any other diseases while in custody of Blue Dog David. Blue Dog David shall exercise reasonable care while in custody of the pet(s). The owner agrees that Blue Dog David and staff will not be liable for any problems that develop provided that reasonable care has been given. The owner grants Blue Dog David permission to provide veterinary care for pet(s) as Blue Dog David deems necessary during pet’s stay. In this event the owner assumes full financial responsibility for any cost incurred. The owner agrees that Blue Dog David may proceed without owner’s consent if they believe it is necessary for the pet’s welfare. In any other event, it is expressly agreed by the owner and Blue Dog David that Blue Dog David’s liability shall not exceed the lesser of the current chattel value of the pet of the same species or the sum of $200 per animal. The owner hereby grants a lien on the pet(s) to Blue Dog David for all charges incurred from boarding and daycare, as well as any other services rendered. If any charges are left unpaid Blue Dog David may automatically exercise it’s lien right fourteen (14) days after failure to pay and or/fourteen (14) days after the owner’s failure to pick up pet(s). Fees shall continue to accrue to the owner after Blue Dog David has enforced lien rights stated herein. Notice of lien will be served by registered or certified mail and be posted to the address of the owner stated in this document. If a pet has not been picked up within ten (10) days of date stated in notice to the owner, the dog will be assumed to be abandoned. If legal action is necessary to enforce the terms of this agreement, Blue Dog David shall be entitled to reasonable attorney’s fees, costs, and necessary disbursements in addition to any fees which said parties may be entitled. By signing this agreement the owner agrees to pay all costs and charges for special services requested, veterinary costs, all damages that the pet may do to persons, property, and other pets, and any other costs related to the pet. It is understood by the owner that all subsequent visits are to be covered in full by the terms agreed to herein. I have read and agree to the terms of agreement:
