A heart condition where you should only do physical activity recommended by a doctor.

Lose balance because of dizziness or ever lose consciousness.

A bone or joint problem that could be made worse by a change in your physical activity participation.

Uncontrolled asthma (i.e. asthma that is not easily controlled by an inhaler).

Currently being prescribed any medication for your blood pressure or a heart condition.

Been pregnant or given birth in the past two years.

Know of any other reason why you should not undergo physical activity? This might include diabetes, a recent injury, or a serious illness.

This does not apply to my medical needs. I will discuss my medical needs with the instructor.

I believe I require a doctor's approval before starting sessions.
