NOVEMBER 21
• Men's Beginners — ₱350 per player
• Women's Beginners — ₱350 per player
NOVEMBER 22
• Semi-Intermediate — ₱650 per player
• Mixed Beginners — ₱350 per player

Call time: 2:00 PM
Tournament starts: 3:00 PM onwards
IMPORTANT REMINDERS
• One registration form per doubles team. Only one player needs to complete the form for both partners.
• Double entry is NOT allowed. Each player may participate in only one division.
• Registration fees must be paid via GCash before the tournament.
• Submitting this form does not automatically guarantee a slot. Registration is confirmed only after our team verifies your details and payment.
• Slots are limited. Registration may close once a division reaches capacity.
