Sweet View Optical & Jambos are looking for volunteer models of different ages to participate in our "Giving Back Looks Good On You" Charity Fashion Show on Saturday, November 21, 2026.

~Official Runway Show: approximately 1:30 PM

Models will showcase curated eyewear from Sweet View Optical while helping us benefit Jambos and its mission of providing brand-new pajamas to kids and teens in foster care across Georgia.

No modeling experience is required. We’re looking for personality, confidence, community spirit, and people ready to have some fun.

Selected volunteer models will also receive an exclusive Sweet View Optical eyewear perk as a thank-you for participating. Details will be shared with confirmed models.

Please note: Completing this form expresses your interest in participating. It does not guarantee runway placement. Selected models will receive a separate confirmation with arrival time, wardrobe instructions, and additional event details.

Wardrobe

We are currently planning for models to wear a simple solid-black base outfit so the eyewear can take center stage. Final wardrobe instructions will be provided to selected models.

Models should generally plan to provide their own clothing unless otherwise arranged by the event team.

I understand that the Giving Back Looks Good On You Fashion Show is a public community event where photography and video recording may occur.

I authorize Sweet View Optical, Jambos, and their authorized event photographers, videographers, media representatives, and event partners to photograph and/or record the participant named on this form during the event.

I understand that photographs and recordings may be used for purposes including event promotion, social media, websites, printed materials, press or media coverage, event recaps, and future promotion of Sweet View Optical, Jambos, and/or Giving Back Looks Good On You.

I understand that no compensation will be provided for the use of these photographs or recordings.

If the participant is under 18, the parent or legal guardian completing this form grants this permission on behalf of the minor participant.

I understand that participation in the Giving Back Looks Good On You Fashion Show may include activities such as walking, standing, wearing eyewear, interacting with attendees, and moving through an active public event environment.

Participants are expected to follow instructions provided by Sweet View Optical staff and event organizers, including instructions regarding runway movement, eyewear handling, backstage or staging areas, and general event safety.

I understand that reasonable care will be taken to provide a safe event environment, but participation in a public event involves ordinary risks such as slips, trips, falls, accidental contact, or property damage.

I voluntarily choose to participate, or authorize the minor named on this form to participate, with an understanding of these ordinary risks.

I understand that eyewear provided for modeling remains the property of Sweet View Optical or its vendors unless otherwise stated and must be handled according to staff instructions and returned following use.
