Mask request form

Hey there! We’re Olympia Mask Bloc, we offer free high quality masks (respirators) to individuals in Thurston County. If you're able to pick up masks, we have a free mask distro monthly at DECAY (402 Washington St NE). View our website for more info. DECAY is ADA accessible (step-free entrance + interior, ADA restroom with grab bars). It is located about a block from the downtown transit center. Street parking is free on weekends. We are a very small group of disabled volunteers, thank you for your patience. What type of masks are you hoping to get?

N95s have headband straps, KN95s and KF94s have ear loops. N95s provide the best protection.
