Anxiety Check-in

This short check-in is based on a widely used questionnaire (GAD-7) that helps identify common signs of anxiety. It gives you a snapshot of how you’ve been feeling over the past two weeks.

It’s not a diagnosis, but it can be a helpful starting point in understanding your experience and whether additional support might be useful.

There are no right or wrong answers, just answer as honestly as you can.
