Please use this form to submit your grievance statement. Your grievance statement should include:
* Your name (unless you are filing an EEO complaint, you cannot remain anonymous but if your issue is affecting multiple people, your steward can file a class action).
* A description of what happened including a time, place and date.
* Any witnesses to the event
* Any other relevant documentation (such as PS forms or other written documentation from your supervisor or manager)
